I. Introductory Provisions
These Terms and Conditions (hereinafter referred to as the “Terms”) govern the contractual relationship between SANDONIA s.r.o., with its registered office at Kaprova 42/14, Staré Město, 110 00 Prague 1, Czech Republic, Company ID No.: 21835934, registered in the Commercial Register maintained by the Municipal Court in Prague under File No. C 407186 (hereinafter referred to as the “Company”), and a natural or legal person (hereinafter referred to as the “Client”) making use of medical tourism services related to eye surgery.
The Company acts solely as a facilitator in arranging eye surgeries at affiliated clinics in the Czech Republic. The Company is not a medical facility, does not provide medical care or expertise, and assumes no liability for any medical procedure or for the health condition of the Client.
II. Scope of Services
The Company mediates the following services for the Client:
- consultations with an eye clinic,
- scheduling of surgical appointments,
- supplementary services such as accommodation, transfers and assistance (if agreed).
The legal relationship concerning the actual medical procedure arises exclusively between the Client and the clinic. The Company is not a party to this contract.
III. Booking and Advance Payment
The Client is required to pay a non-refundable booking fee in the amount of GBP 85 / EUR 100, which covers administrative and facilitation services.
This booking fee is non-refundable, even in the event of cancellation or failure to attend.
All remaining payments for the medical procedure and related services shall be made directly to the clinic, in local currency, and are subject to the terms and conditions of the respective medical provider.
IV. Client Obligations
The Client is obliged to:
- provide complete and truthful information in the medical questionnaire,
- follow all instructions provided by the medical facility,
- arrange all travel documents, insurance, and sufficient funds to cover all related costs,
- notify the Company at least 14 days prior to the scheduled date of surgery in case of inability to attend for any reason.
In the event of cancellation of accommodation for any reason less than 14 days prior to the scheduled arrival, the Client shall be required to pay a cancellation fee equivalent to the cost of the first night.
V. Liability
The Company accepts no liability for the quality, outcome or process of medical procedures carried out by the contracted clinics.
The Company shall not be held liable for:
- damages resulting from incomplete, inaccurate or misleading information provided by the Client,
- failure to undergo the surgery due to medical or other objective reasons,
- currency exchange rate differences when paying by card,
- any alterations or cancellations made by the clinic,
- travel disruptions, including flight delays or cancellations.
All claims, complaints or disputes regarding the medical procedure must be addressed directly to the clinic where the procedure was performed.
VI. Personal Data Protection
The Client consents to the processing of their personal data, including sensitive health information, by the Company for the purpose of facilitating healthcare services, in accordance with Regulation (EU) 2016/679 (GDPR).
Such data will be shared exclusively with the clinic for the purposes of medical assessment and treatment planning, and will not be used for any other purpose without the Client’s explicit consent.
VII. Final Provisions
These Terms and Conditions are effective from the date of their publication on the Company’s website and form an integral part of the mediation agreement between the Client and the Company.
The legal relationship between the Client and the Company is governed by the laws of the Czech Republic, in particular Act No. 89/2012 Coll., the Civil Code, as amended.
In the event of a dispute, the parties agree to make every effort to resolve the matter amicably. If no agreement can be reached, the dispute shall be resolved by the competent court in the Czech Republic.
